After booking and paying for your visit in PracticeQ, use your private paperwork invitation to complete your medical history, current symptoms, medications, allergies, social history, family history, and applicable reproductive questions.
Upload your identification at the end of your PracticeQ questionnaire. Other supporting documents can be shared through the patient portal.
Please use the private link issued for your appointment when available. Completing paperwork does not create an appointment or confirm a payment. This public website does not receive medical-history or identification submissions.
When you submit your paperwork in PracticeQ, your provider reviews it there. Email notifications can alert the practice without including the medical history or ID in ordinary email.
