**We provide medical consultations to provide Online Attestations of documentation for persons that qualify based on State Statute**

Follow These Steps for Medical Marijuana Card Certifications:

Step 1: Print, Fill Out, and Sign the HIPAA Release Form, give it to your Primary Care Provider or specialist and they will send your medical records directly to our office by fax

Step 2: Schedule an appointment by clicking Book Here. Once you provide the needed information, you will be emailed a link to set up your portal on our EHR (RXNT). MCANN is not federally legal, as such, any consultation related to the certification for MCANN is NOT covered by any insurance or Medicaid/Medicare.

Visits must be paid in full prior to seeing a practitioner.

Step 3: You will upload all your records into your patient portal prior to your scheduled appointment time. The Practitioner will certify your file within 24 hours of your in-person clinic visit.
